What does bloody mucus in cat stool mean

I have a cat with a chronic problem - she is constipated, but also has a chronic problem with mucous in her stool. She gets a little mucous, but also has blood. She is on a special diet, but I am worried. What could this mean? I have seen this before in cats on the internet, but never with mucous.

Is this normal?

The cat is not eating very much, is lethargic and has a lot of mucous in her stool. Is this normal?
